RESUMO

【Objective】 To understand the epidemiological characteristics of COVID-19 in Shaanxi Province from December 9, 2021 to January 20, 2022, and analyze the factors influencing the interval from isolation to diagnosis. 【Methods】 We collected the data of local COVID-19 cases from December 9, 2021 to January 20, 2022 published on the official website of Health Commission of Shaanxi Province. Descriptive statistical method was used to analyze the epidemiological characteristics of COVID-19 in Shaanxi Province. Mann-Whitney U test and Kruskal-Wallis H test were used to compare the differences between groups. The unconditional Logistic regression model was applied to analyze the factors influencing the interval between isolation and diagnosis. 【Results】 The outbreak of COVID-19 in Shaanxi Province started on December 9, 2021 and ended on January 20, 2022. The overall change trend of the outbreak showed an "inverted V" shape. A total of 2,080 confirmed local cases were reported, and the main type of disease was mild, with an incidence rate of 5.26/100,000. Xi’an had the most cases, accounting for 98.69% of the total. The reported cases were mainly concentrated in people aged 21 to 55 years old, with a male-to-female sex ratio of 1.19∶1. The median interval from isolation to diagnosis was 3 days, the shortest interval being 0 day and the longest interval being 21 days. Unconditional Logistic regression model analysis showed that the way of finding cases was the factor influencing the interval from isolation to diagnosis. Compared with the way of isolation of the key population, the way of the nucleic acid screening could reduce the risk of late detection of confirmed cases by 89% (OR=0.11, 95% CI: 0.07-0.16). 【Conclusion】 The way of finding cases is the factor influencing the interval from isolation to diagnosis. In the face of the recent intensification of the spread of Omicron variant in mainland China, accurate and rapid identification and detection of confirmed cases can not only reduce the risk of the spread of the epidemic, but also endeavor more time and initiative for the treatment of patients, which is the key to curbing the spread of the epidemic.

Objective@#To observe the expressional changes in Notch signaling pathway and toll-like receptor 4 (TLR4) and their interactions on the functions of CD14+ monocytes in chronic hepatitis C patients.@*Methods@#A total of 24 treatment-naïve chronic hepatitis C cases and 10 healthy individuals, who visited Shaanxi Provincial People's Hospital from August to October 2017, were enrolled. Selected CD14+ monocytes were stimulated by the Notch signaling pathway inhibitor DAPT or transfected with TLR4 siRNA, and the levels of Notch1, Notch2, Hes1 and Hes5 mRNA were detected by real-time quantitative PCR. TLR4 protein levels and phosphorylation of NF-κB was detected by Western blot. ELISA was used to detect the level of cytokines secreted from CD14+ monocytes. A t-test or paired t-test was used for comparison between groups.@*Results@#The relative expression of Notch1 mRNA (3.97 ± 2.03 vs. 0.91 ± 0.76, P < 0.01) and downstream of Notch signaling pathway (5.96 ± 2.31 vs. 0.99 ± 0.45, P < 0.01), Hes1 mRNA and Hes5 mRNA (4.31 ± 1.05 vs. 0.84 ± 0.20, P < 0.01) in CD14+ monocytes of chronic hepatitis C patients was significantly higher than that of healthy individuals. The relative expression of TLR4 mRNA (5.14 ± 1.09 vs. 1.27 ± 0.39) and protein level in CD14+ monocytes of chronic hepatitis C patients were significantly higher than those of healthy individuals (P < 0.01). An inhibition of Notch signaling pathway with DAPT had reduced the relative expression level of TLR4 mRNA (2.58 ± 1.36 vs. 4.34 ± 1.88, P < 0.05), protein expression and phosphorylation of NF-B in CD14+ monocytes of chronic hepatitis C patients. Furthermore, the secretion level of MCP-1 [(94.32 ± 23.59) pg/ml vs. (64.07 ± 9.39) pg/ml, P < 0.01] and IL-8 [(12.54 ± 4.89) pg/ml vs. (7.92 ± 3.01) pg/ml, P < 0.05] was significantly reduced. TLR4 siRNA transfection reduced the expressions of Notch1 mRNA (2.09 ± 1.72 vs. 3.73 ± 1.75, P < 0.05), Hes1 (2.87 ± 0.84 vs. 5.54 ± 0.97, P < 0.01), and Hes5 (2.89 ± 0.93 vs. 4.51 ± 1.54, P < 0.01) in CD14+ monocytes of chronic hepatitis C patients.@*Conclusion@#Interaction of Notch signaling pathway with TLR4 can promote the function of CD14+ monocytes in chronic hepatitis C patients.

Objective To evaluate the necessity and feasibility by using two different PCR-based techniques for prenatal diagnosis of thalassemia. Methods 509 specimens for prenatal diagnosis of thalassemia were detected respectively by single tube multiplex PCR(STMP),reverse dot blot(RDB)or probe melting curve assay(PMCA)for commonα-thalassemia orβ-thalassemia mutations in double-blind tests. Samples with different detection results were confirmed with DNA sequencing analysis. Prenatal diagnosis of thalassemia results were verified or followed up after birth. Results In detectingα-thalassemia andβ-thalassemia,there was one case in STMP + RDB and another case PMCA indicating differentiating results. The detection sensitivity of STMP + RDB was higher than that of PMCA,and its difference can be used as an indication for maternal blood contamination. Conclusion The two PCR methods with different principles were necessary and feasible for the prenatal diagnosis of thalassemia. The two methods were complementary to each other ,which can ensure the reliability of the prenatal diagnosis results and reduce the defects of single technique. It is worthy to be popularized in clinical application.

Objective To compare the reliability of erythrocyte parameter mean corpuscular volume(MCV)and mean corpuscu-lar hemoglobin(MCH)for screening thalassaemia trait.Methods A fresh venous blood sample with the cut-off value of MCV was sent to 21 hospitals of Zhuhai city for conduct the full blood cell analysis within 1 d.Then the inter-and intra-coefficient of varia-tion (CV)as well as bias values of MCV and MCH were calculated and compared.In addition,10 EDTA-anticoagulant venous blood samples were divided into two parts,the effects of stored temperature and time on MCV and MCH were observed.Results The coefficient of variation(CV)of MCV (4.1%)was significantly greater than that of MCH (2.8%)among 21 laboratories,the qualification rate of MCH detection results was 100%,which was significantly higher than 66.7% of MCV (P 0.05).When these samples were stored under the refrigerated condition for 72 h,MCV had no statistically sig-nificant difference compared with the instant detection results of MCV (P >0.05),when stored at the room temperature for 48 h, MCV was significantly increased (P <0.05),MCV had statistical difference in storage for 48 h between the room temperature and the refrigeration.Conclusion Among laboratories and under different temperature conditions,the reproducibility of MCH is better than that of MCV and is more stable than MCV.MCH as the clinical first-line screening for thalassaemia is more reliable than MCV.

The expressions of estrogen receptor (ER) α,ERβ1,and ERβ2 in thyroid carcinoma,thyroid adenoma,and adjacent normal thyroid tissue were determined by RT-PCR and immunohistochemistry,and the relationship of expression levels of three ERs and the main clinical pathologic parameters were evaluated.The result showed that ERα mRNA expression in papillary thyroid carcinoma(PTC) tissues was significantly higher than adjacent normal tissue,while,ERβ2 mRNA expression in PTC tissues was significantly lower than normal tissue ( P<0.05 ).The positive expression of ERβ1 and ERβ2 in the thyroid carcinoma was related with the histological variant of carcinomas; the ERβ2 expressions were related with the lymph node metastasis and TNM staging of the carcinomas.These results suggest that the decreased ERβ2 expression may induce proliferation and carcinogenesis of thyroid follicular epithelial cells as well as progression of the carcinoma,and is a powerful prognostic indicator.

Objective To research the expression of the estrogen receptor alpha ( Erα) and estrogen receptor beta ( Erβ) in uvea tissues of the female rats, and to provide molecular biology data for further studies of the relation estrogen to uvea diseases. Methods Twenty-two adolescent SD female rats were selected. All rats were killed by dislocation of cervical vertebra, the eyeballs were in paraffin imbedding and made to a series of sections, using immunohistochemical method;Erα and Erβ distribution were investigated in uvea tissue of rats;and quantitied by Tanaka scores analytical method. The uteri of rats was used as positive control and PBS as negative control. The level of estradiol in serum of the rats were examined by radioimmunoassay. Results The expression level of Erβ was moderate or highter in stroma cell, anterior pigment epithelium as well as pasterior pigment epithelium of the iris, unpigmented epithelium, pigmented ciliary epithelium and vascular endocemet of the choroid layers. But Erα was not obviously expressed in uvea tissues. The expression rate of Erβ was higher than Erα in these tissues(P<0.05). Immnoreactivity positive substance was granule, which was distributed in the cytoplasm or in the nucleus. The level of estradiol in serum of the rats was (22.13±3.54)ng/L.Conclusion The expression of either Erα or Erβ in uvea tissues of rats is mainly in Erβ. The results indicate that uvea tissue is regulated directly by estrogen throught Erβ.

Objective To investigate the expression and clinical significance of the estrogen receptor(ER) subsets ER?,ER? and Ki-67 in tissues of different thyroid diseases.Methods The pathological data of 93 patients with thyroid diseases were collected from Jun.2003 to Jun.2007,including thyroid carcinoma(56 cases),thyroid follicular adenoma(13 cases),nodular goiter(14 cases) and normal thyroid tissue(10 cases).S-P immunohistochemistry was performed to determine the expressions of ER?,ER? and Ki-67 in the thyroid tissues.Results ER? and ER? were expressed in the plasma and nuclei of thyroid epithelial cells,and ER? was also expressed in the stroma.The positive rates of ER? expression in cases with thyroid carcinoma,thyroid follicular adenoma,nodular goiter and normal thyroid tissue was 87.5%,69.2%,42.9% and 30.0%,respectively,with significant difference between carcinoma and goiter or normal thyroid tissue(P0.05).Conclusion ER? and ER? may play an aggravating effect on cell proliferation in the disease course of the thyroid carcinoma.The expressions of ER? and Ki-67 are related to invasion and metastasis of thyroid carcinoma.

Objective To explore the localization and distribution of estrogen receptor alpha (ER ?) and estrogen receptor beta (ER ?) in retina tissues of normal female rats. Methods Twenty-two SD female rats were killed by dislocation of cervical vertebra, the eyeballs were paraffin embedded and made to a series of sections which were then studied by means of immunohistochemical method. The uterus samples of rats with positive ER were used as positive control, and PBS as negative control. The serum estradilum levels of rats were examined by radioimmunoassay. Results A few specimens were found to be ER ? positive expression in retina pigment epithelium (RPE), ganglion cell layer and nerve fiber layer; while in other regions the ER ? negative expressed. The ER ? evidently expressed in different retina regions which were detected mainly in RPE and nerve ganglion cell layer as well as nerve fiber layer, and secondly in internal plexiform layer, while few ER ? expressed in other regions. Immunoreactivity positive substances were granule in shape which were located in the cytoplasm of pigment epithelium, or in cytoplasm, or in the nucleus of ganglion cell, or diffuse distributed in relative regions of retina.
